Failures during isolation and reinstatement are one of the main causes of loss-of-containment incidents — and may lead to major accidents. This trainer teaches the decisions, not the table of contents: observe the plant, interpret the state, decide, act, and see what the plant does next.

Safety constraint. This is a training demonstrator based on HSG253. It does not replace company isolation procedures, site-specific risk assessment, your permit-to-work system, competent technical judgement, or authorised isolation personnel. HSG253 guidance is followed, not compulsory — but where this trainer states a requirement, it is traceable to the source document.
